Patient retention is a critical factor in achieving positive healthcare outcomes. Across healthcare systems, “gaps in care” lead to increased costs, poorer patient health, and greater strain on care teams. This project explores how modern data analytics can be used to re-engage patients effectively, streamline outreach, and empower care teams with actionable insights.
As part of my degree program at Western Governors University, I am building this project to integrate both academic learning and real-world problem-solving. While the specifics of my dataset and models will be shaped by upcoming coursework, the overarching goal is to create a scalable, ethical, and data-driven re-engagement framework.
